Office Admin Bremer stated they will send an action plan to ensure proper ratio is being followed at all times. Plan will be provided to LPA by due date of 5/1/26. Staff stated they were out of ratio when they were in the playground with more than 24 children and 2 staff. LPA observed staff juggling around to cover ratios. Record Review showed facility was out of ratio between Feb 2-6, 2026. This poses a potential risk to health and safety of children in care
