A procedure is not implemented to ensure that the number, names, and whereabouts of children in care is known to assigned child care workers at all times. Names of children observed leaving the playground with a staff member to go inside the school to use the bathroom remain on the tracking clipboard while they are inside and the staff member taking them inside does not have a list of the names of the children she is taking inside.

The program has exceeded group size repeatedly over the course of the 2025-2026 school year when mixed age groups of school-age children were cared for in one group including the visit date when 43 children were observed cared for in a group.
