Upon a review of the medical log book, it was discovered that there were four incidents since April 2026 where children received head injuries while in care of the center and parents were not immediately notified as is required by rule. These head injuries occurred on 4/10/26, 5/18/26, 5/20/26, and 5/29/26.

It was observed during the monitoring visit that 2 cords are hanging over a crib where children are placed to sleep in the Infant Nap Room, and were positioned in a way where they could be accessible to children. It was also observed that the curtain rod was detached from the wall, and the curtain was hanging over the edge of a crib in way that could be accessible to children and potentially harmful if pulled down by a child.

It was observed during the monitoring visit that plastic crates containing children's water bottles were stacked in front of the back door for approximately one hour before being taken outside with a group of children. When staff were asked, it was discovered that there was not a dedicated storage area other than in front of the door in which these crates are typically stored. Rule requires that all exits and passageways shall remain unobstructed by furniture or other objects.

Rule requires that all sleeping surfaces for children under one year of age shall be placed either at least 2 feet apart, or end-to-end with a solid partition separating them. It was observed during the monitoring visit that several cribs in the Infant Nap Room were placed end-to-end, but did not have a solid partition to completely separate the sleeping surfaces.

Staff A has worked at the center for more than 6 months and was missing a certificate from The Registry nor does she have a record of her educational qualifications to document that the person is qualified as a teacher. Children in the infant room did not have a teacher providing supervision on 7/24/25 when one assistant teacher (Staff F) and one staff with an inaccessible file (Staff H) were providing sole supervision to this group of children. On 7/24/2025, center tracking procedure was not followed when Staff A was unaware of the number of children that she had in care on the outdoor playground. 251.055(2)(e) · Mixed Age Group Of Children Over Age 2 - Group Size
Staff-to-child ratio was not met on 7/24/2025. According to DCF 251.055(2)(e), the number of children in a group may not exceed the maximum number of children that can be cared for by 2 staff members. Staff-to Child ratios were not adjusted on a prorata basis as determined by the staff-to-child formula in the 2y+ room during the monitoring visit. There was one qualified teacher for 27 children, with the numerical weight of the group being 2.604, requiring an additional qualified teacher per group of children. 3 children were sent home, however the numerical weight was still too high at 2.26.
