Each child was not supervised by a child care worker who is within sight and sound of the children to guide behavior and activities, prevent harm and ensure safety when on April 28, 2026 a 3-year-old child was left unattended on the playground for approximately 5-8 minutes. Additionally, on May 7, 2026 a 3-year-old child was unsupervised on the playground for approximately 8 minutes. The center self-reported the incidents.
