The licensee failed to report damage to the premises that may affect compliance with this chapter and any incident at the premises that results in the loss of utility services within 24 hours. During the historic August 2025 flooding, the basement was reported to have had ankle deep water resulting in damage to the basement floor covering (linoleum) which required its removal. The center also lost power during the flooding. The department learned of the damage during the monitoring visit.

A heavy wooden closet door in the daycare nap room that is off its hinges is observed in front of the closet and falls on Licensor Slatten during the visit. Multiple cords including cords for the television, chargers, a baby monitor, and a chest freezer are accessible to children in multiple licensed childcare rooms.

Records of meals and snacks served to children are inaccurate. The licensee reported "tacos, shells, spanish rice, corn on the cob, peaches and milk" was served for dinner on 9/17/25 however the meal record states "baked chicken, green beans, carrot sticks rice and milk" was served.
