Fruit flies are observed in the basement child care including multiple fruit flies on a bowl that contains unwashed sippy cups and a baby bottle containing leftover milk. The bowl has been placed on top of an unwashed high chair tray which also contains fruit flies.

The meal record for the lunch served is not accurate for the date of the monitoring visit. Per the report of the licensee, ham sandwiches, corn, apples and milk were served for lunch however the meal record states "beef ribs, white bread, white potatoes, grapes, milk" were served for lunch.

Staff A is meeting the staff-to-child ratio for the 3 children in care upon and prior to the department's arrival however she has not recorded her arrival time.
