Based on staff interviews, staff members failed to adequately supervise all children to guide behaviors, ensure safety and prevent harm when, on June 1, 2026, a 4-year-old child pulled down the pants and underwear of another child. The 4-year-old child then touched and slapped the bare bottom of the other child, who was lying on a cot. The staff members present did not see the incident at first and were alerted to the incident by other children in the classroom. This incident was self-reported by the center.
