Each child was not supervised by a child care work who is within the sight and sound of the children, when on February 4, 2025, as the classroom was transitioning from the gym to the classroom, a four-year-old child left the group and went back in the gym. The child was unsupervised in the gym for approximately 3 minutes. This incident was self-reported by the center.
