    The director self reported on February 13, 2026, child care worker(s) failed to supervise a three-and-a-half-year-old child when the child was left outside for approximately one minute in the fenced in play space. A high school teacher/students on the adjacent side walk found the child and returned the child to the front door of the center.

This resulted in a three-and-a-half-year-old child being left outside for approximately two minutes. The workers were not aware of the child's whereabouts until the child was returned to the classroom.
