All employees in regular contact with children did not obtain a current certificate of infant and child CPR when Staff A last completed the training on 10/06/2023.

Children had access to hazardous items when children were walking in the parking lot and staff abruptly moved a toddler because the they were close to a vehicle that the staff reported might be "hot, and it could hurt him".

The permanent enclosure has an open area greater than four inches when there was an opening in the enclosure from the ground to the bottom of the fence that was four and a half inches which is considered an entrapment.
