Documentation of the test results for lead levels required for a private well was not available to the Licensor at the time of the monitoring visit. The center shall have the well tested for lead every 5 years using a laboratory certified by the DNR. The laboratory report shall be available to the department upon request.

250.09(4)(b) · Infant & Toddler - Diaper Changing Surface - Disinfection

Applicable rules under s. DCF 250.09(4) shall apply to child care workers when children 2 years of age and older require attention for diapering and toileting. When diapering a child over the age of 2, the provider stated she was not using a 2-step process to clean and disinfect the diaper cleaning surface, and the soap and water and disinfectant spray bottles were empty.
