Each child was not supervised by a child care worker within the sight and sound of the children to guide the children's behavior and activities, prevent harm and ensure safety when, on 10/22/24 a six-year-old child left the cafeteria and was gone for approximately five to ten minutes. Staff were not aware the child had left until the child's parent came to pick up the child. While staff searched for the child, another adult in the building returned the child to the program. The center self-reported this incident.
