The Licensor observed two children under the age of 7 years exited their classroom and they were in the hallway for less than 1 minute before returning back to their classroom. The child care worker was in the classroom, and the child care worker was not within sight of the two children being in the hallway.

The metal dome climber is 4 feet high at the center of the dome and this equipment did not have 9 inches of energy absorbing surface material below its fall zone.

The medical log book in the Rising Star classroom had no signature or initial of the person making the entry for entry date 4-27-2026. On page 10, three lines were skipped at the bottom of the page and new entries are made on the next page.

On 9-29-2025 at around pick-up time, a school-age child had checked out of care and was about to leave the center with his parent. When the child tried to remove his backpack from the wooden cubby located out in the hallway, the wooden cubby fell on top of him. The wooden cubby had been moved away from the wall earlier that day to have the walls painted. When the wooden cubby got moved back against the wall, it was not anchored back to the wall to ensure safety.
