Staff members failed to follow indoor equipment manufacturer recommendations, which could compromise the safety of children in care when a 5-year-old child fell from a toddler's slide and sustained an injury.

Children were unsupervised when a child was injured while in their care, and staff members did not know how the accident happened until they viewed camera footage. The lead teacher was in a different room while the teacher assistant was playing with other children.

An opened closet with potentially harmful materials to children was accessible for infants and toddlers. Items labeled keep out of the reach of children were accessible when diaper rash lotions were stored in an unlocked cabinets.

The premises was not maintained in good condition when at the Little Dino's Room there was a filtration at the ceiling.

The kitchen helper did not wear an effective hair restraint while she was preparing the meal.

A staff member was observed roughly grabbing a child's hands to keep him seated and prevent him from running out of the room. The director was advised to provide the staff member with resources on alternative child guidance techniques.

Some indoor equipment and furnishings were unsafe and accessible to children in several areas of the center. An interior loft was in poor condition, a shelving unit was broken, and sensory materials and mirrors were broken and falling off the wall.

Toddler's development and routine documentation was not updated every 3 months.

251.09(4)(a)10. · Infant & Toddler - Diapering Lotions, Powders, Salves

The directions of diapers lotions, powders or salves on use by children were not posted in the diaper changing area.
