110-300-0241(11) · Large area rugs or installed carpet must be cleaned at least once every six months, or when visible dirt or stains are present, using a carpet shampoo machine, steam cleaner, or other method that minimizes the exposure of children in care to pathogens and allergens. (a) An early learning provider must not use dry shampoos or dry chemical sanitizers or disinfectants, unless approved by the department. (b) If caring for infants, a provider must either place a safe and clean material over large rugs or carpet, or clean rugs or carpet at least once per month or more often if visible stains are present.

1. The carpet in the living room is stained and one infant was playing on the foor periodically.

110-300-0166(3)(c) · To ensure a safe exit from the premises during an emergency, early learning providers must comply with the following requirements: (c) Exit doors must not be partially or entirely blocked.

1. The emergency exit pathway leading to the emergency exit door in the kitchen was blocked because a pressure gate was installed and being used in the kitchen doorway during child care hours.

110-300-0145(6) · Licensed outdoor play areas must be enclosed with a fence or barrier that is intended to prevent children from exiting and discourages climbing. If the outdoor play area is enclosed by a barrier that is not a fence, the barrier may be a wall constructed with brick, stone, or a similar material.

1. The emergency exit gate outdoors does not close automatically and uses special knowledge to open. 2. Currently there is no shade for children. 3. Currently there is not a variety of play equipment outdoors. 4. The outdoor fence area in not finished with a 48' fence. 5. There is a section of the fence panels which do not meet a minimun of 48" high and children can climb onto the roof of the parking area. 6. To ensure a safe environment for children in care, an early learning provider must comply with the following electrical requirements: (b) Outlets near sinks, tubs, toilets, or other water sources must be inaccessible to children or be tamper-resistant and equipped with a ground fault circuit interrupter (GFCI) outlet type;

1. The french doors do not have a method to prevent children from walking into the glass and the door does not open easily. 2. There are nails protruding from the the wooden fence. 3. The door handle must be of the type that can be opened from the inside without the use of a key, tools, or special knowledge, and must automatically unlock when the knob or handle is turned;
