At the time of the site visit there was a loose garden hose hanging off of the fence on the playground, a loose cord on the light table, a loose cord hanging off of the front counter and down next to a napping cot, and another loose cord on the front counter which were all accessible to the children. At the time of the site visit there was a rubber made plastic kitchen that has broken plastic window in the pretend oven area of the kitchen. At the time of the site visit the site was using a large square fan sitting on the front counter and hanging part was over the counter making it a falling hazard. At the time of the site visit there was a plastic structure on the playground with chipping paint, climber with chipping paint, wood fencing with chipped paint, preschool room has shelving with chipped paint, front counter has chipped paint and is warped in spots, some chipped paint on the classroom walls in areas and the window seal of the preschool room, chipped paint on the wall next to the diaper changing table and a hole in the same area the size of a pencil eraser. The site has some outlets in the preschool room that do not have automatic shutters that only allow electrical plugs to be inserted (tamper resistant). The bathroom outlet is not and equipped with a ground fault circuit interrupter (GFCI) tamper resistant outlet.
