There was no documenation that the handbooks had been signed by parent/guardian. There was no documentation that the staff reviewed the disaster plan handbook. Drill log observed was completely blank did not have the year for the form either. There were no monthly inspections, monthly checks, monthly fire and evacuation drills nor quarterly drills documented on the sheet. According to enrollment form for children some started in January of this year.
