During the on site inspection visit for the complaint, during the interview with the site supervisor Ria Basaliza, the licensor was informed that the substatute working that day was not in their provider MERIT account and they do not create employment records for substatutes in their Provider MERIT account. A person names "Don" arrived during the on site visit to start his shift from 4 to 6 pm and Licensor Rossfeld later did a review of the providers MERIT page and found they do not have an employee names "Don" listed in their MERIT account.
