110-300-0165(3)(g) · An early learning provider must take measures intended to prevent other hazards to children in care in early learning program space including, but not limited to: (g) Equipment in poor condition. Equipment in poor condition (loose parts, rusty parts, flaking paint, or other dangers) must be repaired, removed, or made inaccessible to children.
3)g) The toilet seat in the restroom on the right in classroom 5 has peeling paint along the back of the underside of the seat. There is moisture creeping up under the paint and the seat is not in good condition. There are 3 trikes in the main playground that are missing the handlebar grips and the metal bar ends are exposed, which could injure or puncture a child if the trike fell over while riding it. There was also one trike with a broken bell that had a piece of plastic sticking out that could cause injury if a child fell forward toward the handlebars. The caterpillar crawl tube has a missing plastic piece that is needed to hold one of the ends on at the top. It also has cracked/broken plastic at the top of the other end and is not in good condition.

5)b) The exit door to the outside in classroom 4 did not make a sound when it was opened. After further investigation, it was noted the door chime was not turned on. The staff turned on the door chime and this issue was addressed during the inspection. The door chime in classroom 2 on the door leading to the hallway was not working today.

110-300-0146(1)(a)(b) · Playground equipment and surfacing used by an early learning provider must comply with applicable CPSC guidelines, as now and hereafter amended including, but not limited to, installing, arranging, designing, constructing, and maintaining outdoor play equipment and surfacing. (a) Climbing play equipment must not be placed on or above concrete, asphalt, packed soil, lumber, or similar hard surfaces;(b) The ground under swings and play equipment must be covered by a shock absorbing material (grass alone is not an acceptable) such as: (i) Pea gravel at least nine inches deep; (ii) Playground wood chips at least nine inches deep; (iii) Shredded recycled rubber at least six inches deep; or (iv) Any material that has a certificate of compliance, label, or documentation stating it meets ASTM standards F1292.
1)a) There were two small toddler slides, with steps up to the slide, in the grass on the toddler playground. This type of equipment is considered climbing equipment and requires protective ground surfacing to be in compliance with CPSC guidelines. The slides were removed during the inspection.
