(i) Paper, rubbish, or other combustible materials must be at least three feet from furnaces, fireplaces, or other heating devices; (ii) Furnaces and other heating devices must be inaccessible to children in care; and (iii) An appliance or heating device that has a surface capable of burning a child or reaching 110 degrees Fahrenheit must be inaccessible to children in care unless a program activity involves such an appliance or device and children are being actively supervised.

110-300-0146(1) · Playground equipment and surfacing used by an early learning provider must comply with applicable CPSC guidelines including, but not limited to, installing, arranging, designing, constructing, and maintaining outdoor play equipment and surfacing.
Climbing dome on wood chip ground coverage without six foot fall zone as required by CPSC guidelines. Dispute Description: During the inspection on 12/29/2022, the climbing dome was deemed safe and was not moved, unlike the other playground equipment that required relocation due to either the drop space or needing placement inside the playbox. However, I was informed that the playbox on the infant side needed to be expanded to meet the 6-foot drop space required for two 40-inch slides. The preschool playbox expansion was overlooked and not mentioned. All playground items were removed on 12/29/2022, and photographic evidence was sent to the licensor as proof. Had I been instructed to remove the dome or expand the preschool playbox, it would have been done simultaneously with the toddler playbox expansion. Additionally, we added new wood chips on 10/24/2023. On October 26th, we relocated the dome to a new site and added 12 yards of pea gravel to comply with the request for a 6-foot drop zone.

Holes in Preschool bathroom walls from previous screws in wall. Staff R.B. reported to Licensor that a child had pulled on soap dispenser the morning of this visit, causing it to come off wall. Soap dispenser has been placed back onto as of 11/02/2023 based on photo that was sent to Licensor showing soap dispenser secured to bathroom wall. Dispute Description: The plastic storage bin, which had a minor crack at the bottom corner, has been removed and replaced with a small wooden shelf. The flaking paint in the toddler room has been fixed, and the wall has been wiped clean. The holes in the preschool room were caused by ongoing repairs and rearranging of shelves for safety reasons the previous day. The licensor came to inspect on the day the repairs were being made. Unfortunately, a child pulled the soap dispenser off the wall that morning. I explained the situation to the licensor and assured her that it would be fixed that day. I also informed her that the holes in the walls would be repaired once the wood chips were done. 110-300-0455(3) · An early learning provider must keep daily staff attendance records for each center classroom or family home program. These attendance records must be on paper or in an electronic format and clearly document: (a) The name of each staff member (including staff assigned to care for children with special needs and one-on-one care) and volunteers; (b) The number of children in each classroom or family home program; (c) The staff-to-child ratio; (d) The date; and (e) Start and end times of the assigned staff or volunteers.

After speaking with the Licensor on December 22, I was informed about the Sign-in In Sheet. I informed her that I was not aware of the form and that I would type it up that day. I was told it had to be a sheet for all staff to sign in and out to keep track of their time with the children. I created a binder with the Sign In/Out Sheets to go from classroom to classroom. We have used this binder since the implementation. At the inspection on 10/24/2023, I was asked about the forms, and I showed her the book we have been using. She asked if there was one in each classroom, and I told her that the book goes around to the three classrooms when it is time for the staff to punch in and out and take their breaks. I told her I created the form based on what I was told on 12/29/2022 and that I needed her to tell me exactly how the form should look. I requested an example of the form. I want to express my deep concern about the unannounced visit and the communication with the licensor. The experience was confusing and discouraging, which left me feeling frustrated and dissatisfied. I believe that clear and effective communication is essential for a successful partnership, and I would appreciate your attention to this matter. Thank you for your understanding.
