No medication authorization form was completed by the parent for a child with a prescribed EpiPen. Dispute Description: there is an authorization form and note that was written for school and dad added child care on it and was signed and dated for 12/1/25. I can provide that if requested. So I have a note that specifically written for the EpiPen by a doctor and was provided to me dated and signed by the dad. This not was with the special need papers that was printed at a time. 110-300-0215(3)(a)(ii) · An early learning provider must administer medication to children in care as follows: (ii) Nonprescription oral medication. Nonprescription (over-the-counter) oral medication brought to the early learning program by a parent or guardian must be in the original packaging.
