5.4.1.1 · The licensee shall ensure that each child, except school-age children, has an individual crib, port-a-crib, cot, mat, or bed that is safe for the child's age and development, in good repair, and used by only one child each day. No child shall nap directly on the floor or on furniture not designed for sleeping. Cribs or port-a-cribs shall be used for children under twelve (12) months of age. Children shall not nap in cribs or port-a-cribs if they are physically able to climb out unassisted.
Upon arrival to the infant room known as the Little Sprouts, Child A and Child B were sleeping on rest mats that were partially folded to provide angled props on either side of each child. Child A is 3 months old. Child B is 5 months old. Child C and Child D were also asleep on rest mats which were laid flat on the floor. Child C is 10 months old. Child D is 9 months old. Staff A stated that all children enrolled in the Little Sprouts room are under the age of 12 months and all consistently sleep on rest mats. When prompted by this licensor, Staff B moved Child A and Child B to cribs. Child C and D woke shortly after this licensor's arrival.

3.4.7.3 · Within six (6) months of the initial date of employment, staff working with children and the program director shall maintain an up- to-date BFIS Quality and Credential Account. Documentation, verification of qualifications and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to NLCDC to be verified and maintained in BFIS.
