5.1.2 · General Health Examinations: Within forty-five (45) days of enrollment, the licensee shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.

5.4.3.2 · Children shall not be required to sleep. Children who do not nap shall be provided space and equipment for quiet play.
It was determined through staff statements that the program has children lay on their rest mats for 30 minutes or more, without opportunity for quiet activities during that time. It was confirmed that children must lay on the mats for a minimum of 30 minutes, even if they do not nap. This is a violation because children shall be offered the opportunity to have quiet activities during rest if they are not sleeping.

7.3.1.1 · The licensee shall ensure and maintain documentation that the program director is at least twenty-one (21) years of age and meets the qualifications below based on the licensed capacity of the CBCCPP.
The program is licensed for 18 children. After transcript review of director qualifications, it was determined that the Director has not completed requirements to meet the additional qualifications required for a program licensed for 13 or more children, as stated in rules 7.3.1.3 and 7.3.1.5. This is a violation because it is required to have a 3 credit course in Program Management or Staff Supervision and a 3 credit course in managing an early care and education program that includes budgeting and financial management.

6.2.7.2 · Staff's expectations of children's behavior and responses to children's behavior shall be appropriate to each child's level of development and understanding. Guidance shall be designed to meet the individual needs of each child.
Staff member X was observed to have placed her legs across a one year old child's body on a cot while the child was crying, after they would not settle for rest time. When asked, Staff Member X admitted that they had placed their legs across the crying child in order to apply pressure to the child. Staff member X did not take additional measures to calm or soothe the child and utilized their cell phone during the interaction. This is not a developmentally appropriate response.
