6.2.6.6 · The program director shall ensure that no person be left alone with children without approval from the Division based on the results of a background check that includes fingerprinting; with the exception of a parent may be left alone with his/her own child(ren).
A Classroom Aide, hired by the local Supervisory union to work in the program supporting a child one on one, had been working in the program daily for the past six (6) months without completing the background clearance process. This staff member was alone with children during the visit.

3.4.7.3 · Within six (6) months of the initial date of employment; staff and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations, and the program director shall maintain an up-to-date BFIS Quality and Credential Account. Documentation, verification of qualifications, and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to Northern Lights at CCV to be verified and maintained in BFIS.
Upon review of staff BFIS Quality and Credential accounts and on-site staff files, it was identified that two (2) out of three (3) staff were missing required documents. Documents provided on site included First Aid/CPR certifications for the Director and Teacher Associate. The substitute was missing documentation of their qualifcations, current Pediatric First Aid/CPR training, their Annual Professional Development hours, and Mandated Reporter training.

8.2.2 · The PSPP is exempt from rules 3.4.5.2 and 3.4.5.3 of these regulations, however the licensee shall maintain a complete and up-to-date list in the PSPP's BFIS Account of auxiliary and partner staff that shall contain, at minimum, the following information: Name, date of birth, home address, and telephone number; Signed statement verifying understanding of legal requirement to report suspected child abuse or neglect; and Documentation of a current Records Check Authorization form.
During the site visit, it was discovered that five (5) Partner staff worked with children in the program more than five (5) times in the past six (6) months and were not on the Associated Parties list in the program's BFIS Quality and Credential Account.
