3.4.7.5 · The licensee shall maintain a current list of the licensee or designee, staff, auxiliary staff, and volunteers as in the rules in section 7.7.5 of these regulations, as applicable in BFIS. Any changes shall be reported through BFIS within five (5) working days of the change.
There were four (4) individuals listed as staff on the associated parties list who were no longer working at the program. During last years compliance visit twelve (12) individuals were listed on the associated parties list but were no longer employed at the program. A violation was not cited but the report notes that Licensor Irwin reviewed this regulation and advised that when a staff person leaves the program they must go into BFIS and enter an end date.

5.10.1.1.2 · Children shall be protected from choking hazards presented by cords on window coverings, electric or electronic appliances or equipment and telephones.

5.1.2 · General Health Examinations: Within forty-five (45) days of enrollment, the licensee shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.
A review of children's files reflected that none of the five reviewed files contained General Health Exam forms. All five of the children had been enrolled at the program for at least 45 days.

5.10.1.10.3 · All poisonous or toxic materials, except materials required for routine cleaning, shall be locked in a secure storage area.
Upon arrival at the program, the door to the utility closet in the Preschool room was open. The closet contained unfinished sheetrock and insulation, large bottles of laundry detergent and large containers of disinfectants labeled "Keep Out of Reach of Children" that were at level where children could reach. Additionally, the cupboards under the kitchen sink in the preschool room did not have child safety locks. The cupcoards contained bleach products and shaving cream labeled "Keep Out of Reach of Children". In the school age room, the door was open to the utility closet. This closet contained large containers of bleach products, disinfectants and paint thinner which were labeled "Keep Out of Reach of Children". All products were at a level which children could reach. The cupboards under the kitchen sink in the School Age room did not have child safety locks. The cupboards contained dinifecting wipes, peroxide, rubbing alcohol and other first aid lotions and supplies. These items were labeled "Keep Out of Reach of Children". There were no children present in the school age room during this visit.
