5.11.6.4 · Infants shall be held for bottle-feeding until they are able to hold an unsupported bottle. At no time shall a bottle be propped for feeding an infant.
An infant, confirmed by staff and observed during this visit by Licensing Field Specialist Crawford as unable to hold their own bottle, had a bottle propped into a cloth holder for feeding while seating in a bouncy seat. The staff person providing care in the group did not assist the child with the bottle or hold the child during the feeding.

6.2.1.3 · The program director shall ensure that the number of staff present meets requirements described in the chart below for ages of children, maximum group size, and staff/child ratios at any time of day.
From approximately 2:20 p.m. to 3:12 p.m., the program had six (6) children between the ages of 4 months to 15 months of age and three (3) children ages 18 months to 2 yrs of age in care with two staff persons present in the building and providing care. Staff to child ratio required a third staff person to be present and providing care. This is a serious violation of staff to child ratio requiring a Parental Notification Letter.

5.10.1.1 · General Safety: The licensee shall ensure the physical facilities of the CBCCPP, grounds, equipment, and toys present no hazard to the health, safety, and well-being of the children.
A child in the infant/toddler group reached behind a storage cabinet and grabbed an ant trap with visible powder bait. Staff were notified by this licensor that the child had this and it was immediately removed from the child's grasp. The child was taken immediately to wash his hands. Program staff reviewed all areas of the floor behind and beneath items in the children's spaces and confirmed no additional traps were present.
