4.4 · A system for taking attendance, including documentation of the time when each child arrives and departs each day he/she attends the Afterschool Child Care Program, shall be established.

4.5 · Programs shall implement a system that documents the whereabouts of each child in attendance at all times.
Upon arrival at the program an announcement was made they'd be leaving shortly to go on a field trip. Staff B was asked if they were walking or taking transportation; they stated they use their bus and make a few trips in order to get everyone to the other location. When asked how it is documented which children remain at the program, and which children are trasported, Staff B stated they do not document this. Staff B confirmed that children are chosen to load into the bus if they are ready, and not necessarily by group. Staff B was asked if it was documented when children arrive at the alternate location; they stated no. Staff B confirmed that the attendance documents the arrival and departure of children but does not capture whereabouts of children as they move about their day.

11.14 · The Licensee shall ensure garbage, recycling and compost is not allowed to exceed the limits of its container and is removed from rooms used by children at least once the container is full, if it develops an odor, or presents some other health or safety risk.

15.23 · When a dishwasher is not available and single service items are not used; a three- compartment sink with adequate space for drain boards shall be required for the dishwashing, rinsing and sanitizing of dishes and utensils. Dishes shall be washed in hot, clean water at 120 degrees Fahrenheit (49 degrees Celsius), rinsed in hot, clean water and immersed for 2 minutes in sanitizing rinse. Bleach solution shall be used following the instruction of the manufacturer regarding sanitizing. Hand washed dishes shall be air-dried.

18.77 · Within six months of the initial date of employment, Staff working with children and the Site Director and/or Program Administrator shall maintain an up-to-date BFIS Quality and Credential Account. In an office utilized by the program there is a massive amount of mouse and rat droppings upon the floor, on a desk surface, inside a desk drawer, and on shelving. The same floor has a large size rodent trap and a pile of food consisting of saltine crackers, candy corn, popcorn seeds, and uncooked pasta. Program staff report the office space has not been cleaned for weeks as they just haven't gotten around to cleaning this room. 13.15 · Hard floors and carpets shall be cleaned at least daily. Carpets shall be vacuumed daily. Carpets shall be hot water extracted at least twice per year.
In an office utilized by the program there is a massive amount of mouse and rat droppings upon the floor, on a desk surface, inside a desk drawer, and on shelving. Also, upon the same floor is a pile of food consisting of saltine crackers, candy corn, popcorn seeds, and uncooked pasta. Program staff report the office space has not been cleaned for weeks as they just haven't gotten around to cleaning this room. There are mouse droppings on the floor in an art room and behind the refrigerators located in the dining space.
