6.2.6 · Supervision of Children Staff shall ensure visual supervision of children, except as allowed in rules 6.2.6.1-6.2.6.4 of these regulations.
On 08/26/2025 the program contacted the Child Development Division and reported that Child A who is twenty- six(26) months old, left the classroom and was found outside on the porch. The program reported that footage from a video camera captured the child on the porch without an adult for five(5) minutes. The video was not saved and not availble during this licensing visit. Conversations with staff indicate that after Child A was finished washing their hands, the staff member in the bathroom area thought Child A headed into the rest time area. In fact Child A walked around the cubbies and to the exit door because a gate in the classroom was not secured and the cubbies created a blind spot because of where staff were positioned.. Child A was able to flip the lock mechanism on the door and exit onto the porch. The director was looking out of a window near the infant room and saw Child A sitting on the porch attempting to put their shoes on.Staff within the classroom were not aware that Child A was not in the room. The gate on the porch was also not secure. Had Child A left the porch they would have direct access to the church parking lot and North Main Street. In response to the incident the director met with staff and reviewed supervision and safety protocols. Installed an alarm on the exterior door. Staff have increased the frequency of counting children and looking at each child's face while counting and are ensuring gates are shut by the last staff member entering a gated space. A serious violation is being cited based on the hazards that Child A had access to when they were outside without an adult.

5.4.2.7 · No positioning devices shall be used unless required by the child's health care provider. Instructions for the use of any such device shall be kept in the child's file in writing from the child's health care provider.
On 07/08/2025 the program submitted a self report to the Child Development Division explaining that program administrator's learned that staff in the infant room were propping the mattresses in the cribs. Staff were doing this for infants who were exhibiting respiratory illness. Staff were not provided with medical notes from the families that would allow them to prop the mattress. In the self report and confirmed by this licensor, this practice had occured over several months. Infant safe sleep regulations prohibit this practice unless the families provide a doctor's note allowing for the infant to be propped during sleep.

5.4.2.8 · Infants shall not be placed in cribs or port-a-cribs with bottles. Pacifiers are permitted in cribs and port-a-cribs with parental permission. Pacifiers may not have cords or clips that may pose a strangulation risk.
Staff statements reflected that three (3) infants regularly use a pacifier for sleep while in cribs. Upon review of children's files it was identified that parental permission had not been obtained to permit pacifier use in cribs. Staff statements reflected unawareness of this requirement.

3.7.2.2 · The licensee shall ensure that evacuation drills are conducted at least once a month, and children and staff are evacuated in under three (3) minutes. Licensees of a CBCCPP located within a public or independent school building may count a lock-down drill performed while the CBCCPP is in operation in place of a monthly evacuation drill with the CBCCPP children and staff and at least three (3) of the monthly drills conducted within 365 days shall be evacuation drills.
Upon review of the program's evacuation drill log it was identified that the program had not completed evacuation drills during the months of December, January, or February. Staff statements confirmed this.

6.2.6.6 · The program director shall ensure that no person be left alone with children without approval from the Division based on the results of a background check that includes fingerprinting; with the exception of a parent may be left alone with his/her own child(ren).
Staff A was observed alone with two (2) infants in the infant nap room for mroe than five (5) minutes. Staff A has not received approval from the Division to be left alone with children.
