After a review of staff BFIS Quality and Credential accounts and staff files at the program, it was determined that five (5) staff employed longer than six (6) months were missing items from their BFIS Quality and Credential account. Items ranged from CPR or First Aid certifications, proof of qualifications, or required trainings. This is a repeat violation.

3.4.7.3 · Within six (6) months of the initial date of employment, staff working with children and the program director shall maintain an up- to-date BFIS Quality and Credential Account. Documentation, verification of qualifications and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to NLCDC to be verified and maintained in BFIS.

8.2.1 · Notwithstanding onsite staff documentation for application for employment, references, and written annual performance reviews from rule 3.4.5.1, the licensee shall maintain a legally notarized affidavit on site, signed by the licensee or designee, testifying that each staff file contains these records as required in the rule 3.4.5.1 of these regulations. The licensee shall permit representatives of the Division to have full access to off-site records to verify documentation.

7.1.2 · First Aid and CPR Staff who are counted in the staff/child ratios shall obtain training in pediatric first aid and infant and child CPR within six (6) months of beginning work in the CBCCPP and remain currently certified.
Staff certifications for CPR and First Aid were not available for review during the site visit. When asked, staff indicated that their CPR and First Aid certifications were not active and had expired. Due to lack of evidence in BFIS and confirmation from staff statements regarding lack of First Aid and CPR Certifications, this is a violation. Staff must maintain CPR and First Aid certifications and be certified within three (3) months of hire per updated memo.

5.1.2 · General Health Examinations: Within forty-five (45) days of enrollment, the licensee shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.
It was determined that the program was not collecting general health exam forms for enrolled children at the program. This form is required within 45 days of enrollment for each child that is enrolled at the program.
