5.10.1.10.4 · Materials required for routine cleaning shall be stored and used in a safe manner out of the reach of children.
On 03/24/2026 this licensor was at the program and noted that cleaning supplies were stored on the kitchen floor. The Director stated that children do not have access to the gated kitchen. The Director was asked to move them in case a child opened the gate, or the gate was left open, as children would have access if this occurred. On 3/31/2026 this licensor visited the program and noted that the kitchen gate was open and the cleaning materials were still on the kitchen floor which made them accessible to children in care.

5.10.4.5.3 · The licensee shall ensure that the CBCCPP is kept free from rodent and insect infestation.

5.10.4.5.6 · Exterior stairs, walks, ramps, and porches shall be cleared of ice and snow or other environmental hazards that may block exits.
The doors labeled exit by the mechanical room and toddler rooms could not be open, the provider stated that the doors were locked. An inspection by the Division of Fire Safety revealed that a 4 foot high pile of snow and ice were blocking the doors. Once the snow removed you could open the door.

5.10.1.9.5 · The licensee shall ensure that pest baits are not to be used unless in childproof bait boxes. Bait boxes shall be inaccessible to children.
On the floor in the mechanical room were green balls later identified as Ramik Green, the label on the bag gives this warning PRECAUTIONS: KEEP OUT OF REACH OF CHILDREN, PETS AND LIVESTOCK. May be harmful or fatal if swallowed or absorbed through the skin. Rubber gloves must be worn when disposing of dead rodents, unconsumed bait and empty containers. The room was unlocked and accessible to children. The director stated that they had put the Ramik Green down after finding rodent feces.

5.10.1.1 · General Safety: The licensee shall ensure the physical facilities of the CBCCPP, grounds, equipment, and toys present no hazard to the health, safety, and well-being of the children.

3.7.2.2 · The licensee shall ensure that evacuation drills are conducted at least once a month, and children and staff are evacuated in under three (3) minutes. The pre-k classroom contained 2 puzzles, 2 sets of manipulatives, 3 dolls, an abacus, around 15 blocks, a shoe box size of magnetic tiles, a kitchen set with a few foods with seven children present. The toddler room contained a minimum variety of material. The program director stated that she could only find building toys.

5.11.6.1 · Staff shall ensure parents of each enrolled infant provide written feeding instructions specifying the child's primary source of nutrition (breast milk, infant formula, or both) and feeding schedule. The instructions shall be updated as the child's diet changes.
