5.6.7 · A record of the administration of all medications shall be made including medication dosage, time of administration, name of staff administering, and any adverse effects observed. These records shall be maintained for 365 days from the start date of medication administration and may be kept in the child's file or in a medication administration log.

2.3.9.3 · The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.

A review of the staff's BFIS quality and credential accounts, staff files, and the support from Northern Lights to find missing documentation, it was determined that five (5) staff members employed longer than six (6) months did not have an up-to-date BFIS Quality and Credential account as required and was missing education documentation, CPR and First Aid certificates and required training certificates.

7.2.4 · The licensee shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.
During the visit, it was observed that Staff A, who had not yet received fingerprint supported background clearance from the Division, was left alone with a child in the bathroom for approximately seven minutes. Additionally, it was observed that Partner Staff A was left alone with a child for approximately four minutes in the classroom and Partner Staff B was left alone with a group of children in the classroom for approximately four minutes. Partner Staff are not required to complete fingerprint supported background clearances or required safety trainings and therefore may not be left alone with children.

5.10.1.10.4 · Materials required for routine cleaning shall be stored and used in a safe manner out of the reach of children.
During the visit, it was observed that a first aid kit containing iodine, burn gel and other products labelled "Keep Out Of Reach of Children" was stored in a low, unlocked cabinet within children's reach. It was also observed that a plastic tub of cleaning wipes labelled "Keep Out of Reach of Children" was stored in a low, unlocked cabinet within children's reach.

7.1.2.2 · Staff who are counted in the staff/child ratios and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations shall obtain training in pediatric first aid and in infant and child CPR within three (3) months of beginning work in the CBCCPP and remain currently certified.

7.2.1 · The licensee shall ensure that any new individuals regularly present at the CBCCPP submit a Records Check Authorization form to the Division prior to working with children in the CBCCPP.

3.4.6.1 · The licensee shall maintain a record of the date of each evacuation drill as required in the rules in section 3.7.2 of these regulations that includes the number of children and staff evacuated, time taken to evacuate, and notation of the drill conducted while children were sleeping or resting. Evacuation records shall be kept for two (2) years. When the CBCCPP counts lock-down drills as allowed in the rules in section 3.7.2 of these regulations, the licensee shall maintain a record of the date of each lock-down drill, the number of CBCCPP children and staff who participated, and note that the drill performed was a lock-down drill.
The last evacuation drill documented was September 22, 2020. Staff report that evacuation drills occur every month and they acknowledged that the required information was not documented at the time of the drills.

Upon review of staff BFIS quality and credential accounts and discussion with the program director, it was found that staff had missing or incomplete documentation in their accounts and additional individual was no longer with the program. Staff A has not been in the program since September of 2020. Staff B, C and D are missing Mandated Reporter certification, Staff C and D are missing the VT orientation training certification and staff D is missing Emergency Response training certification. Staff B, D and E are also missing their educational documentation. All staff have been employed by the program for more than six (6) months.
