5.1.2 · General Health Examinations: Within forty-five (45) days of enrollment, the licensee shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.
During a review of children's files, it was noted that 75% of the files reviewed, did not contain General Health Exam Forms. The Director confirmed that all of the children whose files were reviewed have been enrolled for at least forty-five (45) days. The Director stated that while she has requested these forms from families on multiple occasions, after her recent review of children's files, approximately fifty (50) percent of the enrolled children did not yet have General Health Exam forms on site.

7.4.4 · All staff shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations. Substitutes filling a staff position for less than thirty (30) consecutive days within a 365 day period shall be exempt.

Staff A confirmed that staff A, B, C, and D had not completed 15 hours of annual professional development based on individual start dates due to the stress that the COVID-19 pandemic placed on staffing.
