6.2.6.1 · Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
The staff members interviewed stated the program does leave an infant inside sleeping while the group of infants are outside on the playground. The program had a protocol to check on the infant every ten (10) to fifteen (15) minutes and had a monitor on the infant. During this licensing visit an infant was noted to be inside sleeping in their crib from 9:10 to 9:25 alone while the group was outside on the playground. The preschool teacher in the preschool classroom and the teachers outside cannot hear or see the infant and cannot physically respond to the infant immediately which is in violation to this rule.

2.3.9.3 · The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.
A staff member did not submit a new record check authorization form or submit to fingerprinting prior to their expiration date of 10/11/2024. An annual assurance letter was sent to the program on 5/26/2024 that had this staff listed stating they needed to submit a new record check authorization form and submit to fingerprinting before the expiration date.

5.10.3.2.1 · All outdoor play equipment shall be sturdy, safe, clean and free from rough edges, sharp corners, pinch and crush points, splinters, and exposed bolts. To prevent entrapment, no openings in equipment shall be between 3.5 and nine (9) inches. Chains on swings shall have protective coverings and swing seats shall be made of soft materials with no sharp edges.
The large wooden climbing structure has a plastic twisty slide that is cracked at the top and some of the bolts are coming out of the wood. The crack has sharp edges where children may place their hands. Children were using this slide during this licensing visit.

5.10.1.1.3 · Furniture and equipment shall be sturdy. Furniture and equipment that present a tipping or falling hazard shall be secured.
In the preschool/toddler bathroom, there is a plastic shelf that is over five (5) feet tall that was not secure and posed as a tipping hazard. This shelf is located next to one of the toilets used by children and had supplies consisting of large bottles of vinegar, hand sanitizer, a create filled with supplies, a large bin and other paper supplies. This shelf moved easily when touched with two fingers.

6.2.4.3 · The program director shall ensure that each group of children is led by a staff member who is at least qualified as a teacher associate for at least 75% of the hours of operation.
During the compliance visit, there were three groups of children and only one group was being led by a qualified teacher. The other two groups were being led by teacher assistants. Staff statements and a review of staffing schedules indicate that there are three to four groups of children per day with only one to two staff members meeting teacher or teacher associate qualifications. It is required that all groups of children present on any given day are led by a staff member that is qualified as a teacher or teacher associate.

5.10.1.9.5 · The licensee shall ensure that pest baits are not to be used unless in childproof bait boxes. Bait boxes shall be inaccessible to children.
Two pest baits were located in the lower food storage cabinets next to the sink in the preschool room. One pest bait was located in a drawer next to the sink in the preschool room. The cabinets and drawer had no childproof lock making them accessible to children.

5.10.1.1 · General Safety: The licensee shall ensure the physical facilities of the CBCCPP, grounds, equipment, and toys present no hazard to the health, safety and well-being of the children.
A bottle of bleach and dishwashing detergent was in an unlocked cabinet under the sink in the preschool classroom where children could access them. Two bottles of bleach and a power tool were in an unlocked cabinet under the sink in the infant room where children could access them. Kitchen chopping knives were accessible to children in two unlocked drawers in the preschool room. The window in the infant room was cracked on the inside glass pane. The blue dramatic play shelf in the preschool room located next to the loft had knob-like legs and was a tipping hazard.
