A toddler left the playground behind the building without staff seeing the child leave. The child went to the front of the building. The child was near a tree next to the front door of the building, between the building and the driveway of the program, where the child was seen by a teacher from the infant classroom. It is not clear how long the was gone from the playground. This is the fifth incident of children being unspervised by staff since October 2021.

On 10/19/2021 at approximately 4:25 p.m., Child A left the playground and entered the classroom to use the bathroom. Child A was found in the bathroom by Staff member Z, who was leaving for the day and came into the classroom to unlock the door between the playground and the classroom. The staff on the playground supervising Child A's group (Staff Y and Staff X) did not realize that Child A had left the playground until notified by Staff member Z. Staff Z and two other staff from a separate classroom had been on the playground and also did not see the child leave. On 11/2/2021 at approximately 4:05 p.m., it was discovered that Child B had been left in the classroom without supervision following a trip in from the playground to use the bathroom with Staff W and another child. Staff W took Child B and another child in to use the bathroom. Another staff member, Staff V, was in bathroom with a child and offered to stay for the additional two while Staff W took the first child outside. Staff W stated that they would stay with the children and Staff V went back outside with the first child. Staff W and a child returned to the playground. Staff W, Staff V, and the other classroom teacher on the playground, Staff U, did not realize that Child B did not return to the playground. After several minutes, Staff V noticed the classroom door open a bit and Child B inside. On 11/3/2021, at approximately 10:40 a.m., Child C was observed by an adult as the child walked out of the front door of the program. The adult immediately returned Child C inside to Staff T, who returned Child C to the classroom and stayed in the classroom temporarily. Staff R and Staff S were supervising a classroom of 11 children when a physical altercation broke out between two children. When Staff R and Staff S intervened, Child C left the classroom without an adult noticing. Staff Q was also assigned to that classroom for the day and had just left on break.

5.4.1.8 · Bedding used by a child shall be stored in a manner that prevents contact with bedding used by other children.
Nap mats in the Junior Toddler room were stored wtih sheets on the mats and the sheets of one child were touching the sheets of another child. Also, in Pre-K 1/Preschool the nap mats were placed on a table with sheets on the mats prior to placing them on the floor. While on the table, bedding was touching other bedding.
