11.27 · Essential Maintenance Practices (EMPs) shall be performed prior to initial licensure and annually thereafter in the space defined as licensed space. A review of staff BFIS accounts and staff file documents reflects that many documents have not been submitted to Northern Lights at CCV to be uploaded staff Credential accounts in a timely manner as required. At least seven staff members who have worked with the program for six months to four years, had documents missing from their credentialing accounts. While the program administrator submitted many documents to Northern Lights at CCV following this visit, as of 06/11/2025, she was not able to provide evidence of previous document submission. .

5.40 · Before the expiration date on the card, training for First Aid and CPR shall be validated by the American Red Cross, American Heart Association or other equivalent, state- recognized organization, and the specific certification documentation maintained in the appropriate Afterschool Child Care Program Staff file.
A review of staff BFIS credential accounts and documents from staff files reflected that six (6) staff, who have been working with the program for between three and eighteen months, have either not completed certifications or have not renewed their certifications prior to the date of expiry for approved infant and child CPR and pediatric First Aid.
