6.2.6.1 · Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
On 6/13/2025, a child was left inside the childcare program unsupervised for approximately a minute or two while their assigned teachers and classmates were outside for snack. The child walked away from their group at the door leading outside of the Voyager room, walked through the preschool room and into the motor room where they were found playing alone. The program reported this incident to the Child Development Division. A violation is being cited due to the child's age, the length of time they were missing and the breakdown of systems that let a child wander away.

6.2.6.6 · The program director shall ensure that no person be left alone with children without approval from the Division based on the results of a background check that includes fingerprinting; with the exception of a parent may be left alone with his/her own child(ren).

The program, upon request to review the staff schedule provided Licensing staff with two (2) systems of accounting; a staff schedule and the Bright Wheel system which staff sign in and out of classrooms documenting their arrival and departures with each group of children. This along with staff statements found that staff who had not been cleared to be alone with children were alone with children on 10/9/2024 and 10/14/2024, which is a violation of this rule. During the debrief with Sara LeBlanc and Tatianna LeBlanc they presented another document that had daily changes of the staff schedule called the Daily Schedule. This document did not show which classrooms or time the house support or support staff worked in the classroom, which ultimately did not confirm documented times cleared staff were paired with non-cleared staff.

5.6.4 · Written permission from parents to administer medication must include all of the following information for each prescription and non-prescription medication administered to children: * Name of the child; * Child's date of birth; * Any medication allergies; * Name of medication; * Dosage to be given; * Time when medication is to be given; * Route of administration (mouth, ear, nose, topical, inhalation); * Reason for the medication; * Start and end dates for administration of the medication; and * Any special instructions.

5.6.5 · All prescription medication administered by staff shall be in its original container, properly labeled for the child, and administered prior to the expiration date on the container.
A child's epi pen was not in the original box properly labeled with the child's name or had the label with the epi pen. The epi pen had also expired in April 2024 and staff were unaware that it had expired.
