13.20 · All prescription medications shall be kept in the original containers and be clearly labeled with children's name, dosage, medication name, and schedule of administration.

5.14 · The licensee shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.
The Two (2) program staff working during this licensing visit, were left alone with children, and had not completed the fingerprint process which includes receiving the results. The letter the program received states they cannot leave staff alone with children until the results of the fingerprints are confirmed and received by the program.

5.39 · Within six months of hire, all paid Staff counted in the staff/child ratios shall have obtained CDD-approved training in infant/child CPR and basic first aid for children, injury prevention and emergency readiness. All Staff shall receive retraining in first aid prior to the expiration date on each Staff person's first aid card.

3.1 · The Licensee shall be responsible for compliance with all state and federal laws pertaining to the safety and well-being of children in licensed care.
It was confirmed during this licensing visit that the program did not submit the lead water test bottles to the state to complete the lead water test for the program. There is a condition on the program's license for the lead water test to be completed by 6/30/2023.

5.35 · The Program Administrator or designee shall ensure a minimum of 8 hours of annual Professional Development Activities for new and returning Afterschool Child Care Program Staff related to the attainable goals defined in each Staff's Individual Professional Development Plan (IPDP). This training is in addition to any infant/child CPR certificate or recertification and First Aid training.

13.18 · Written parental permission shall be obtained before administering any medication. The permission statement shall include the name of the child, name of the medication, dosage, the date and time to be given and how the medication is to be given (mouth, ear, etc.).
The parent permission form for release of medication was not filled out by the parent and was not with the child's medication or in the child's file during this licensing visit. The child had an epi pen and other medication in the program's emergency bag.

5.5 · An Afterschool Aide that works in an Afterschool Child Care Program shall be sixteen years or older and be in eyesight or ear shot of an Afterschool Child Care Program Staff when the Aide is sixteen or seventeen years of age. There shall be one adult Staff member over the age of eighteen assigned to supervise no more than two Afterschool Aides, age sixteen or older.
The program staff member left two (2) Aides who were sixteen (16) and seventeen (17) years old with a group of eleven (11) children for approximately twenty (20) minutes while she went inside with one (1) child during this licensing visit. The program staff was out of eyesight and ear shot of the two aides.

7.1 · Each child shall be supervised at all times by Staff as appropriate for the age of the child.
A child left the group she was assigned to and went into the office without staff's knowledge. The staff did not look for this child, but staff came across the child in the office when they went to get their own lunch boxes. The child was in the office for approximately four (4) to five (5) minutes alone before staff arrived to get their lunch boxes.

10.9 · Emergency evacuations shall be practiced with the children each month. A record of practice drills for the previous and current year shall be maintained and available onsite. Practice drills may be pre-announced. There shall be: * a system in place to assure that all children are accounted for at a predetermined safe place; * a safety plan that may include additional lock down practices and procedures.
The program did not have a fire drill log and staff stated they have not had an evacuation drill in which children exit the building. The program was licensed in April of 2023.
