7.1 · Each child shall be supervised at all times by Staff as appropriate for the age of the child.
On 9/23/2024, a child in care left the gymnasium, went down the hall, out the door and part way down the ramp in front of the school with out staff knowledge. The child was seen by their parent as they arrived for pick up and the child ran back into the program. Staff estimate the child was missing for no more than a couple minutes. The program administrators met with the family after this incident and put a plan together to ensure child safety. This is a serious violation.

4.9.e. · The Afterschool Child Care Program shall report to the Division: Self-reported violations: If an incident or situation occurs in an Afterschool Child Care Program while children are in care that could be considered a Serious Violation as defined in rule 2.44 of these regulations, the Afterschool Child Care Program has a responsibility to self-report this to the Division within 24 hours of the incident. A complete report will include a description of the incident, what the Afterschool Child Care Program has done to verify the particulars of what occurred, and how the Afterschool Child Care Program has taken corrective action to ensure the safety and well-being of children and prevent a future Serious Violation. If such a report is received in a timely manner from an Afterschool Child Care Program with a history of consistent regulatory compliance, no child has been seriously injured or harmed as a result of the incident or situation, and the Afterschool Child Care Program has taken prompt and appropriate corrective action, the Commissioner or his or her designee may, on a case by case basis and at his or her discretion elect to issue a warning, rather than a violation related to that incident or situation.
