7.1.2.1 · The FCCP shall have successfully completed pediatric first aid and CPR prior to being licensed and must remain currently certified.
According to documents on site and in the provider's BFIS Quality and Credential account, their First Aid and CPR certification expired on 11/05/2024. The provider recently completed the online portion of the First Aid and CPR training and is scheduled to complete the in person portion soon. The provider thought the expiration date was in November of 2025. I asked if she had a certificate from November of 2023, as that would be needed to prove that her certification was still valid. She did not have any documentation from 2023.

2.3.8.2 · The FCCP shall complete all annual licensing requirements by the anniversary date of the FCCH current license.
The provider's fingerprints expired on 4/19/2024. The annual assurance letter submitted through the programs BFIS account, dated 10/1/2023 had informed the provider that their fingerprints are about to expire. The provider did not submit a new record check authorization form or complete the fingerprint process prior to this licensing visit.

5.8.3 · The FCCP shall create and file an incident report for each accident, injury or medical emergency that leaves a visible mark or first aid has been administered, even when medical treatment is not required. An incident report shall include the name of the child, date, description of the injury or medical emergency, how it occurred, adult witnesses, first aid provided and medical care required. The child's parent shall be provided a copy of the report at pick up that day. Serious injuries, animal bites and death must be reported to the Division as specified in the rules in section 3.1 of these regulations.
An incident report was not provided to a parent when Child A was injured on 10/29/2019. Lynn confirmed that Child A has been injured on sereval other occations leaving visible marks in the past 6 months, only one accident report was provided to the parent. Lynn stated she does not routinely write injury reports to provide to parent if their child is hurt in her care.

5.8.2 · The FCCP shall ensure that if an accident or injury occurs to a child while in attendance, immediate action is taken to protect the child from further harm and notify the child's parents as quickly as possible.
Lynn confirmed that she did not attempt to notify a parent as soon as possible when Child A was injured at approximately 10AM on 10/29/2019. She provided information about the injury to the person who picked Child A at 4:50PM, and did not talk to the parent until the next morning. She confirmed that there have been other occasions in the past 6 months when Child A has been injured while in care and she did not attempt to notify the parent as soon as possible. She shared that it has been her practice to wait to notify persons picking children up at pick-up time if an injury has occurred during the day unless she thinks the injury is serious and the child may need medical care.
