6.2.6.1 · Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
A four-year-old child left the bathroom unseen by the two teachers working with the eight (8) children present, opened the door to the staircase, walked up a flight of stairs and exited the front door to the porch. The child was outside approximately three (3) to four (4) minutes. A staff member returning to the center, saw the child on the porch alone without a teacher present. She returned the child to the classroom. This is a serious violation that has been previously cited on 6/21/19.

3.4.7.3 · Within six (6) months of the initial date of employment; staff and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations, and the program director shall maintain an up-to-date BFIS Quality and Credential Account. Documentation, verification of qualifications, and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to Northern Lights at CCV to be verified and maintained in BFIS.
After a review of BFIS Quality and Credential accounts, it was determined that staff employed longer than six (6) months did not have an up-to-date BFIS Quality and Credential Account as required and was missing qualification information and required training certificates.

Two children, ages four and five years, left the classroom unseen by the two teachers working with the 10 children present, opened the door to the staircase, walked up a flight of stairs and exited the front door to the lawn and porch. A staff member on break in another part of the building heard the children outside for approximately 8 minutes, unaware that the children did not have a teacher with them outside. As that staff member returned from her break she saw that the two children on the porch did not have a teacher with them and in fact they were alone outside. She returned them to the classroom and reported it to a teacher present. Several minutes later, the same staff member was in the kitchen and heard the stairwell door close shut, so she exited the kitchen, went up the stairs and found the two children outside again on the porch. This is a serious violation.
