5.1.2 · General Health Examinations: Within forty-five (45) days of enrollment the FCCP shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.

During the site visit on 04/07/2021, there were 8 children under school-age and one school-age child in care. This is not in compliance with the charts included for this regulation. A review of attendance records reflected that Bonita had cared for 11 children on 52 days between September 8, 2020 and April 7, 2021. This is not in compliance with rule 6.2.1.2. This is a serious violation.

3.3.3 · The FCCP shall establish a system for taking attendance, including documentation of the time when each child arrives and departs each day he/she attends the FCCH. The FCCP shall save all daily attendance records identifying the hours of children s attendance for at least twelve (12) months from the date that care is provided.
The program's attendance records for 04/07/2021 were not accurately recorded. A review of the attendance records during the site visit on 04/07/2021, reflected that there were 6 children under school-age and 1 school-age child in care. There were observed to be 8 children under school-age and 1 school-age child in care. Child A and Child B, who were both present and under school-age, were not reflected on the attendance record for 04/07/2021.
