In the older children's classroom and stairwell outside the classroom, the wood floors were partially covered. The coverings were damaged and some of the unfinished wood was showing through. In addition, in the older children's classroom, there is an unfinished wooden threshold in the archway between the two (2) rooms that is approximately a foot wide, and some baseboard missing. The licensees said that a flooring contractor was supposed to have come to lay down carpet in the stairwell and finish the floor in the classroom, but the contractor did not show up as scheduled.

5.14 · The licensee shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.
Upon entering the building, a large group of children were present in the Shul with individuals identified as staff who had not yet submitted record checks to the Division. None of the individuals who have been cleared by the Division to work alone with children were present in the room with the children. In the Phase III visit that occurred on 5/29/2025, Licensing Field Specialist Carrie Knight reported that this rule was understood. Additionally, on 5/30/2025, Licensor Knight sent an email to the program detailing this requirement.

7.1 · Each child shall be supervised at all times by Staff as appropriate for the age of the child.
When departing the building's parking lot, Licensor Carrie Knight observed Child X (five years old) alone on the sidewalk near the parking lot without staff supervision. Licensor Knight immediately notified Licensor Robin Rielly, who was driving behind her. As they parked, the child returned inside. Upon approaching the building, Licensors Knight and Rielly observed the exterior door propped open and Child X was standing in the doorway of the interior door, playing with the door. As Licensors Knight and Rielly entered, Child X moved away from the door and hid under a table in the Shul. No staff were monitoring the door at the time and did not see Licensors Knight and Rielly for about a minute after entering the Shul. This is a serious violation.

4.4 · A system for taking attendance, including documentation of the time when each child arrives and departs each day he/she attends the Afterschool Child Care Program, shall be established.
The program was unable to provide attendance that showed the times each child arrived and departed, only the days they attended. In the Phase III visit conducted on 5/29/2025, Licensor Carrie Knight reported that the program intended to utilize a computer application for tracking children's attendance, and that the app will allow for tracking the time of each child's arrival and departure each day.
