During classroom observation, a child had a toileting accident resulting in a puddle of urine on the floor. The puddle was next to a rest mat and another child was asked to lay down on that mat, while the child who had the accident was put down on another mat. Staff were in that immediate area but did not notice or react to the puddle for approximately 5 minutes, when it was ultimately pointed out by licensing after repeated requests for a child to lay on the mat touching the urine. While staff were attending to the child in the bathroom, two other children were observed eating food that had fallen to the floor during lunch time. One child eating food from the floor had a different lunch than their classmates and may not yet have experienced the food they were eating.
