    Evidence: In the two year old room, the door fram in the classroom at the bottom paint is peeling. Outside on the infant toddler play area side, there were multiple areas on the building with peeling paint and door frames with peeling paint.

    8VAC20-780-510-P · Based on record review and staff interview, the licensee did not ensure the authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

    Evidence: 1- Child #1's medication authorization form signed by parent was not renewed every 10 working days. 2- Child #1 medicaiton authorization form for long term was not signed by child's physician. 3- The authorization was good for 10 working days. The medication authorization form for child #1 started February 23, 2024 . Ten working days was March 8, 2024 . 3- The center did not return the medication with in 14 days or have parent fillout another medication authorization for 10 days or get a long term med ication form signed by parent and physician. 4- Fourteen days was March 28, 2024 and medication was still at the center on April 24, 2024. .
