8VAC20-780-40-E · Based on interview and review, the center did not ensure that the center's activities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence: 1. Child Supervision Guidelines states: "Children must be supervised, in the direct line of sight and within ear shot of a staff member, at all times. All state mandated adult to child ratios must be followed at all times. " 2. Center policy also states: "Face-to-Name means staff must see the child's face and say the child's name when confirming all children are accounted for throughout the day. Face to names must be used to document staff to child ratios throughout the day: hourly, children arriving or leaving the classroom, transitioning children from one location to another (playground, fieldtrips, electives etc), combining and or blending classrooms. Face to Name sheets is kept current at all times." 3. The Face to Name sheet for the toddler room did not have Child #1 checked in the classroom. The face to name did not have child #1 checked out of the 2's classroom. Staff #1 and Staff #2 confirmed they did not check child #1 in and out of the classrooms when child #1 transitioned. Staff confirmed name to face sheet was not current. Child #1 was not supervised, in the direct line of sight within ear shot of a staff member for approximately nine minutes.

Evidence: Staff #4 hire date 09/18/2023 the TB results were recieved 09/19/2023. Staff #7 hire date 10/2/2023 the TB results were recieved 10/03/2023 . Which the date of results were recieved were after the date of employment.

Program director confirmed they did not have documentation and complete the sex offender check during the inspection for both staff.

Evidence: During the inspection, the licensing inspector asked to view where the Notice of Intent (NOI) was posted in the facility. A member of management and the licensing inspector went to view the postings located at the front entrance of the building. The NOI was not posted. Before inspector left the NOI was posted with inspection and letter.
