Four children in the Toddler Room were observed participating in a fine-motor skill activity where they used a small spoon to transfer various small-shaped manipulatives into a container. Child A (Age 2 years) who was participating in the activity was observed placing a pearl-sized bead into their mouth with a small spoon. The Licensing Inspector alerted staff A who was supervising the activity of the occurrence. Staff A intervened by removing the bead from the child's mouth and removed the activity from access to the children. There were a total of 12 children in the Toddler Room ranging in ages of 23 months - 3 years old being supervised by 3 staff.

1) At approximately 4:15pm, the whiteboard listed 64 children on the three to six year old playground and 24 children on the toddler playground. Staff interviewed stated that the whiteboard is used to identify the number of children present on the playground. 2) At approximately 4:15pm, the LI observed 62 children playing on the three to six year old playground and 22 children playing on the toddler playground. 3) The stipulation on the license states that only 50 children can be on the three to six year old playground and only 19 children on the toddler playground at any one time.
