1. On 1/20/26 at approximately 12 noon, child A (2 years of age) repeatedly climbed on and jumped off a bookshelf in the classroom. Staff on more than 3 occasions told the child from across the room to discontinue the jumping and climbing on the bookshelf. The child continued the behavior and fell and sustained a gash across their right eyebrow. 911 was called and the child received six stitches. 2. There were 2 staff in the classroom with 12 children. Neither staff approached the child in an attempt to stop or redirect the behavior.
