Evidence: On 5/12/25 in the toddler classroom, ages 16-24 months, Child #1 sustained an injury to the face. Staff #1 wrote a report stating that the child fell on the carpet. It was later discovered that Staff #1 did not see the injury occur and that the injury to the child?s face was a result of being bitten. Interviews revealed that Staff #1 struggled with supervision while multi-tasking.
