On 9/29/25 at approximately 11:00 a.m. the Licensing Inspector observed 12 children ages 4 -5 years old exiting the Pre-K classroom with 1 staff to the playground. The director removed two children from the group and placed them in the two year old classroom. Once the remaining ten children reached the playground, staff A did not follow the center's name to face accountability procedure to account for children when each threshold is crossed.
