8VAC20-780-340-A · Based on interviews and video footage it was determined that staff did not always ensure the care, protection, and guidance, of children while in their care. Evidence: On 10/19/2023 at approximately 9:44 am the preschool one classroom was preparing to go outside. Staff A leads the students out of the classroom to the playground while Staff B follows at the end of the line. Approximately one minute later child A (Age 3) exits the bathroom of the classroom into the empty preschool classroom. The child reportedly looks around the classroom appearing confused. The child then sits at the block area and plays with some toys. The child then goes to the window to look at the other children on the playground. At approximately 10:16 am Staff C walks by the classroom and observes child A alone in the classroom. Staff C retrieves child A and returns them to their class on the playground.

Evidence: The gate on the toddler playground adjacent to the parking lot has a button mechanism accessible to children that allows access to the center parking lot. On 08/23/2022 at 4:45 pm child A (Age 2) was able to exit the toddler playground by simply pushing a button that allowed the child to open the gate and enter the parking lot.
